APPLICATION STARTS ON JANUARY 15 UP TO MARCH 8, 2024!

ONLINE APPLICATION

All Applicants are required to apply via the NASA ISIP ONLINE PORTAL only.

Only those who are currently enrolled for SY 2023-2024 may apply, provided that they have submitted their complete requirements and have passed the screening process. There will be no face-to-face submission of requirements. The requirements list can be found below.

PREPARATION OF REQUIREMENTS FOR SY 2023-2024 AVAILMENTS

You are encouraged to prepare all your requirements prior to going online for the actual application to avoid delays.
Please note that INCOMPLETE online applications will NOT be processed.

LIST OF REQUIREMENTS

Please prepare the following requirements prior to your online application to avoid delays in processing your application.

  • VOTER’S REGISTRATION

    (Please attach your Calaca voter’s registration here) [PDF file only]

    a. For applicants 18 years old & above, applicant’s own voter’s ID or Voter`s Certification is required.

    b. For applicants 17 years old & below, parent/guardian’s voter’s ID or Voter`s Certification is required.

  • REGISTRATION FORM/ENROLMENT FORM FOR 1ST SEMESTER and 2nd SEMESTER of SY 2023-2024

    NOTE:

    MUST BE CERTIFIED TRUE COPY OF SCHOOL REGISTRATION FORM OR SCREEN SHOTS OF THE STUDENT REGISTRATION IN THE SCHOOL PORTAL AND AT LEAST 6 UNITS PER SEMESTER.

  • CERTIFIED TRUE COPY OF LATEST REPORT CARDS SY 2023-2024 [1st SEMESTER]

    Important: Must have no failing or incomplete grades on Final Average Grade for the SEMESTER 1 of the SY 2023-2024.

  • PHOTOCOPY OF LATEST SCHOOL I.D.

    with 3 signatures of applicant or CERTIFICATION OF ENROLMENT from school if latest ID is not yet available.

  • ORIGINAL CERTIFICATE OF ELIGIBILITY

    ISSUED BY THE CITY SOCIAL WELFARE AND DEVELOPMENT OFFICE.

  • 2X2 PICTURE WITH WHITE BACKGROUND

    MOST RECENT 2X2 ID PICTURE WITH PRINTED NAME BELOW / TAKEN WITHIN LAST 3 MONTHS.

  • CERTIFICATE OF PARTICIPATION IN COMMUNITY SERVICE

    All NASA ISIP recipients are required to participate in a community service project or similar activities assigned by the City of Calaca as a pre-requisite for your continued availment of the educational financial assistance. The project will be announced within the term of your application. A certificate will be awarded upon your complete participation of the required project.

ELIGIBILITY FOR EDUCATIONAL ASSISTANCE

ELIGIBILITY

Only applicants who are eligible for the educational financial assistance will be accepted.

  • Must be of good moral character.

  • Must be a permanent resident of Calaca, Batangas for at least six (6) months.

  • Currently enrolled for the school year/semester in any public/private school (Senior High School, College, Masteral, and Doctoral).

  • For Grade 12 Students, College, Masteral and Doctoral, applicant must be a registered voter of Calaca, Batangas.

  • For Grade 11 students or 17 years old & below, applicant’s parent/guardian must be a registered voter of Calaca, Batangas and is a permanent resident for at least six (6) months.

  • Failure to adhere with the principles of the Declaration of Commitment.

THE CERTIFICATE OF ELIGIBILITY WILL ONLY BE GIVEN TO THOSE WHO HAVE NOT VIOLATED ANY OF THE TERMS & CONDITIONS AND DECLARATION OF COMMITMENT FROM YOUR PREVIOUS NASA ISIP DISTRIBUTION OF AID.
THE CERTIFICATE OF COMMUNITY SERVICE CAN BE OBTAINED UPON COMPLETE PARTICIPATION IN THE ACTIVITY THAT WILL BE ANNOUNCED BY THE LGU
